Dokter Miranda Grey (Halle Berry) is crimineel psychologe in de Woodward inrichting voor vrouwen. Hier behandelt ze gevaarlijk gestoorde patiënten zoals Chloe(Penélope Cruz). Miranda’s comfortabele levenverandert op slag als ze op een morgen wakkerwordt in de cel van haar eigen inrichting. Ze herinnert zich niets maar wordt wel beschuldigd van de moord op haar man. Miranda blijft volhouden dat ze onschuldig is, maar als ze last krijgt van angstaanjagende visioenen wordt de grens tussen werkelijkheid en waanideeën steeds moeilijker te onderscheiden.
