John Reilly (Jeffrey Combs), seine Frau Susan (Barbara Crampton) und die gemeinsame Tochter Rebecca (Jessica Dollarhide) treten ihre scheinbar schöne Erbschaft an: eine uralte Burg in Italien. Die Ehe zwischen John und seiner Frau allerdings ist alles andere als gut. Seitdem John alkoholisiert einen Unfall gebaut hat, sind der kleine Sohn tot und die Tochter blind. Bald spürt Rebecca die Anwesenheit von etwas Unheimlichem in dem alten Gemäuer. Doch niemand glaubt ihr. Es kommt zu einer Reihe unglaublicher Vorfälle. In den tiefsten Kerkern der Burg lauert offenbar ein grauenhaftes Monster. Jedem, der dem Castle Freak begegnet, droht ein blutiges Ende. Und viele kreuzen seinen Pfad des Grauens .
